Integrated Neighbourhood Teams

The aim is to bring together different organisations from across health and social care to deliver more joined up care at a neighbourhood level. By simplifying and streamlining services, we will ensure that residents receive the right care, at the right time, in the right place. The first phase will focus on defining neighbourhoods and developing a clear understanding of the roles and responsibilities necessary to place the right professionals in the community, enabling care to be delivered closer to home.
