Integrated Winter Plan

Our integrated winter planning approach led to improvements in patient care. We significantly reduced ambulance handover delays and decreased the number of individuals who were medically ready to leave the hospital but were waiting for social care support.

Several initiatives were launched including:

Dedicated social workers in inpatient mental health settings to provide timely support.
Social prescribing support at our Same Day Discharge Centre to connect patients with community resources.
An enhanced intermediate care scheme to facilitate early discharges for patients.

These efforts ensured that our patients received the care they needed promptly and efficiently.
