Introducing our Integrated Neighbourhood Team Clinical Leads

OneWolverhampton is pleased to share that GP practices, Primary Care Networks (PCNs) and key partners across all neighbourhood localities have identified an Integrated Neighbourhood Team (INT) Clinical Lead.
Integrated Neighbourhood Teams (INTs) bring together professionals from primary care, community services, social care and the voluntary sector to work more closely around local neighbourhoods and populations.
Each neighbourhood will have one Clinical Lead who will provide leadership and help develop a more integrated, multi-disciplinary way of working with partners across health, care and the voluntary sector. These roles are a key step in building effective neighbourhood teams focused on prevention, person-centred care and better outcomes for local communities.
Our INT Clinical Leads are:
- Dr Kam Ahmed – West
- Dr Salma Reehana – South
- Dr Raj Pitchika – East
- Dr Shahid Rafiq – North
The Clinical Leads will work alongside the wider OneWolverhampton INT programme team to help design and embed new ways of working, strengthen multidisciplinary collaboration and support strong clinical governance.
Workshops will be held in each locality during 2026 to bring together partners and colleagues involved in Integrated Neighbourhood Teams. These sessions will focus on:
- Building connections across neighbourhood partners
- Developing a shared vision and purpose for Integrated Neighbourhood Teams
- Beginning to shape how we work together to deliver better outcomes for our communities
