Community Support Teams

A Community Support Team has been working alongside hospital teams to help individuals return to the place they call home as quickly and safely as possible. Their support includes providing practical support such as cooking materials and clothing to support effective discharge and reduce the risk of readmissions.

The team, which has received additional funding from the partnership as part of plans to support hospital flow over winter, meets weekly with the hospital social worker team to identify patients who are ready to go home, but may need some extra support once they get there.
